How to make a referral
We welcome referrals from a variety of referring sources including hospitals, community partners, healthcare professionals as well as self or family referrals. Our Central Registry team is available to help you with questions you may have about Caregiver Recharge services and making a referral.

When you contact the Central Registry, a customer service representative will arrange for you to meet with a Care Coordinator who works on behalf of all Caregiver ReCharge service providers.
The Caregiver ReCharge Care Coordinator will assess whether you and your loved one are eligible based on the combination of assessments, information gathered about you and your loved one and referral priorities. The Coordinator’s decision will be based on established criteria.
Please note that services are not available to people living in retirement homes, long-term care homes and other care facilities.
There is no cost to caregivers for the program. The Caregiver ReCharge Services are funded by the Mississauga Halton Local Health Integration Network (MH LHIN) for caregivers who provide support to loved ones with high needs who live in the communities of Oakville, Milton, Halton Hills, Mississauga and South Etobicoke.
If you and your loved one are eligible for the Caregiver ReCharge Services, your information will be transferred to the Caregiver ReCharge service provider that can best meet your needs.
Your service provider will work with you to develop a service plan that helps put your mind at ease when you leave your loved one in their care.
Your Caregiver ReCharge service provider will tell you the number of respite hours that will be available to you on a monthly basis. A minimum number of hours are to be used each month, unused hours are not transferrable beyond the end of each month.
Once you know that you are eligible for the program, give some thought as to what support you will need on a daily or weekly basis.
