Supports for Daily Living is an innovative regional service for high risk seniors with complex needs. We make it possible for seniors to continue living in their own homes by providing 24/7 access to frequent, urgent and intense onsite personal support to help with the essential activities of daily living. Services are delivered to seniors in their homes by eight (8) designated service providers that meet approved standards for the delivery of high quality Supports for Daily Living services within the communities of Mississauga, Oakville, southwest Etobicoke and parts of Milton.


Service providers approved by Ontario Health carry the Supports for Daily Living Approved Service Provider seal of approval.

Three different models to deliver services:

Designated Buildings
(Hub)

There are several designated buildings throughout Home and Community Care Support Network Mississauga Halton that provide Supports for Daily Living services. Personal Support Workers (PSWs) are located onsite and deliver services to eligible seniors in their individual units, apartments or condos within the building.

Designated Neighbourhoods
(Hub and Spoke)

In designated neighbourhoods, the Supports for Daily Living service provider has an onsite office in a building that services seniors who live there, but also provides services to eligible seniors who live in neighbouring apartment buildings, town homes, condos or homes within close proximity to the building.

This transitional service ensures speedy discharge of high-risk seniors to their homes, regardless of location. Dedicated providers deliver support until stability, coordinating transitions to community partners for continued care. This model is only referred to from the hospital through a priority stream.

Eligibility

High risk seniors who are living in their own homes within the communities of Mississauga, Oakville, southwest Etobicoke and parts of Milton, or who need help to return home quickly upon discharge from hospital.

Cost

Supports for Daily Living is funded by Ontario Health to provide support to seniors (65+), living in the South Etobicoke, Mississauga and Oakville areas.

  • What is Supports for Daily Living?

    The Supports for Daily Living is designed to support high risk seniors by providing multiple daily visits by trained Personal Support Workers.  Visits are prescheduled and are based on individual needs (approximately 1.5 hours per day, 7 days a week, 365 days a year).

  • Who is eligible for Supports for Daily Living services?

    Supports for Daily Living is designed to support high risk seniors, aged 65+, who have complex needs, but who are medically stable. These services are currently available to individuals who:

    • Live in their own private home, or within a residential setting such as an apartment building
    • Demonstrate a need for daily access to personal support services throughout a 24 hour period (with or without homemaking services)
    • Are able to direct their own care or have a substitute decision maker or a live-in caregiver to direct care
    • Are able to communicate their needs (with or without aides)
    • Pose no risk to themselves or others.

    When one's condition or disability increases your dependency on others for support, it can often lead to questions about independent living and whether or not you can still manage living in your own home.


    Supports for Daily Living is a model for community-based assisted living that provides high risk seniors with access to pre-scheduled and/or urgent/on-call personal support, essential homemaking services and safety and reassurance checks seven days a week, 365 days a year.


    Perhaps you have been using home care services offered through Home and Community Care Support Services, but recent changes in your health mean they’re no longer adequate to meet your needs. Transitioning to our services may help bridge the gap between regular community-based home care services and long-term care.


    Non-medical in-home services are provided by trained personal support workers (PSWs) and are customized to meet the individual needs and preferred schedule of each senior. A full range of services is available through scheduled visits and/or on-call urgent response anytime during the day, evening and overnight hours.

  • Can a PSW administer insulin injections?

    As a non-medical program and self directed care we are unable to support insulin injections.

  • What does the Supports for Daily Living program cost?

    Supports for Daily Living is funded by the Home and Community Care Support Services Mississauga Halton for caregivers who provide support to loved ones living in the South Etobicoke, Mississauga, Oakville, Georgetown, Milton and Halton Hills areas.

  • What should I expect during the referral process?

    • Once Central Registry receives your referral you will be contacted to schedule an eligibility assessment with a Central Registry Coordinator.
    • The Central Registry Coordinator will conduct an assessment to determine eligibility for the program and gather the information needed.
    • If you are eligible for the program, your file will be added to a waitlist until there is a vacancy available in the Supports for Daily Living system.
    • While on the waitlist, a Central Registry Coordinator will contact you periodically to update your file.
    • Once there is a vacancy in the Supports for Daily Living system, your file will be sent to an approved Supports for Daily Living provider to develop a customized service plan with you. Once a service plan is in place, your Supports for Daily Living services will begin.

    If you are not eligible for this service our team will help facilitate referrals to more appropriate agencies on your behalf to find a program that best fits your care needs.

  • Is there a waitlist for service?

    Service opportunites are determined by the availability and funding capacity of providers. This can cause a delay in service and a wait time may occur.

  • Who does not qualify for this program?

    Supports for Daily Living is unable to support clients who:                  

    •  Are medically unstable and cannot be left alone between visits
    • Are unable to direct their own care or do not have a substitute decision maker or a live-in caregiver to direct care
    • Pose a risk to themselves or others.
    • Under 65 years of age

Minister’s Medal Honouring Excellence in Health, Quality and Safety

The Supports for Daily Living program was awarded the inaugural Minister’s Medal Honouring Excellence in Health, Quality and Safety in November 2013 for providing community resources to seniors discharged from hospital so they can recover at home rather than a long-term care facility.


The Minister’s Medal recognizes the collaborative efforts of Ontario’s health service providers in improving care.


3M National Award for Health Care Quality

The Supports for Daily Living program was recognized with a prestigious 3M National Award for Health Care Quality at the National Conference of Canadian Health Care Leaders in June 2011.


The 3M Health Care Quality Team Award in the non-acute category recognizes innovation, quality and teamwork. The pioneering Supports for Daily Living initiative has had proven success in reducing hospital stays for seniors, reducing unnecessary ED visits and developing alternatives in the community to avoid long-term care home placement for seniors who don’t need this level of care.
